We provide this Privacy Statement to inform you of our Privacy Policy and practices and about the way your information is collected online and in clinic, and how that information is used.
What personal data we collect and why we collect it
On-line
We collect your name and email address when you choose to send a message via our website message form. This is an automated process linked to our business email in order to reply to your request. None of this information is kept and will be deleted as soon as the request have been dealt with, unless you give us specific authorisation to contact you again.
In clinic
As a qualified and insured practitioner I have a legal obligation to keep accurate records of your personal details such as date of birth, address, telephone numbers, email and relevant medical information relating to your treatment.
It is my duty to ensure confidentiality and safe keeping of this information.
How we take care of your data
All paper records will be held securely in a locked filing cabinet, and notes taken on electronic devices will be password protected. No client files are left on surfaces for other clients/staff to read. All data taken whilst on a mobile treatment will be transported in a locked bag, out of sight in a boot. No notes are left unattended in a vehicle at any time.
How we use your data
On occasion you may be asked for permission for your information to be shared with another practitioner or medical service for referred treatment. Personal data will then be sent via email or postal service separately to your treatment information with an allocated reference code to ensure anonymity.
It might be necessary for me to communicate with you from time to time via email or SMS. This may include Newsletters. An opt-out option will always be available.
No data will be shared to 3rd parties without consented permission, and no information will be sold to 3rd parties for business reasons.
You have access to your records at any time but are not allowed to take them away.
Your records will kept securely for 8 years following your last recorded visit, or until you are 25, if you are under 16 years of age.
